Application Document Requirements

 

In addition to securing approval from the credit card processing Acquiring Bank, the information submitted provides a better understanding of your company's card processing needs and helps us secure the most competitive processing rate possible.

Merchant credit card account document requirements are listed in one of (4) four classifications.  Please review  the classification that describes your company’s Monthly Credit Card Sales Volume, Transaction Type and Average Sales Ticket Amount.   

Along with your completed Credit Card Merchant Account Application; please submit the items listed for your classification.



Credit Card Merchant Account Classifications 

Class = Express Application Online - Approval within 24 hours

Criteria:  For Online e-Commerce merchant web site based business.

Monthly Credit Card Volume = $0 - $10,000.00

Average Sales Ticket = $0 - $ 500.00

Keyed Transactions = Over 30%

Swiped Transactions = N/A

Requirements:

1.    Complete and Submit Online Application

2.    The Merchant Must Have A Website

3.    If Merchant is other than a Sole Proprietor submit a copy of Sales Tax Permit or Certified Articles of Incorporation or Partnership Agreement or Copy of Secretary of State Certificate of Registration, or Copy of Secretary of State Website showing Active Status of Corporation. 

Note:  Approval within 24 hours requires an Online Application that is completed with all required information.  Approval processing time excludes holidays and weekends.

The following Class I - Class III Criteria list the merchant application requirements for all merchants that exceed the Express Application Online requirements.  Contact Us direct if you need assistance.

Class I

Criteria:  Class I application submission requirements apply if your company’s credit card processing fits any of the following descriptions:   

  1. Terminal device “Swiped” Transactions: 

Monthly Credit Card Volume = $0 - $100,000.00 

Average Sales Ticket = $0 - $ 1,000.00

And/Or

  1. Keyed Transactions:

Monthly Volume  = $0 - $25,000.00

Average Sales Ticket = $0 - $ 500.00

Class I Application Requirements:

  1. Complete and Submit Application

  2.  Submit Voided Check - Pre-Printed

Note: If Void Check is not Pre-printed, submit a copy of Business License or Sales Tax Permit or Bank Letter of acknowledgement of your business relationship with the bank.

  1. If Merchant is other than a Sole Proprietor submit a copy of Sales Tax Permit or Certified Articles of Incorporation or Partnership Agreement or Copy of Secretary of State Certificate of Registration, or Copy of Secretary of State Website showing Active Status of Corporation

OR

A Site Inspection Including Interior and Exterior Photos May Be Submitted In Lieu of Item 3 If Monthly Volume Is $10,000.00 or Less.

Class II 

Criteria:  Class II application submission requirements apply if your company’s credit card processing fits any of the following descriptions:  

  1. Terminal device “Swiped” Transactions: 

            Monthly Credit Card Volume = $0 - $250,000.00 

     Average Sales Ticket = $0 - $ 2,500.00

And/Or

  1. Keyed Transactions:

            Monthly Volume  = $0 - $100,000.00 

     Average Sales Ticket = $0 - $ 1,000.00

Class II Application Requirements:

  1. Complete and Submit Application

  2. Submit all Class I documents

  3. A Site Inspection Including Interior and Exterior Photos

  4. A copy of a state issued ID or Passport of the principle

  5. Marketing material example with DBA name promoting a product or service offered

  6. 3 months of previous merchant credit card statements

      OR

Bank statement if no previous card statements are available

Class III

Criteria:  Class III application submission requirements apply if your company’s credit card processing fits any of the following descriptions:  

  1.  Terminal device “Swiped” Transactions: 

Monthly Credit Card Volume = Over $250,000.00

Average Sales Ticket = Over $ 2,500.00

  1.  Keyed Transactions:

Monthly Volume  = Over $100,000.00

Average Sales Ticket = Over $ 1,000.00

Class III Application Requirements:

  1. Complete and Submit Application

  2. Include all Class I & II Documents

  3. Copy of Articles of Incorporation, Partnership Agreement, or Proof of Non-Profit Status

  4.  Most Recent & Signed Two Years Business Balance Sheets & Income Statements

  5. Most Recent & Signed Personal Financial Statements

  6. Most Recent & Signed Two Years Personal Tax Returns

  7. Most Recent & Signed Two Years Business Tax Returns

  8. Most recent three Months Business Bank Statements
